How to Know If You Have a Cavity

Here’s the honest, slightly frustrating answer: in the earliest stages, you often won’t feel anything at all. Cavities don’t announce themselves the way most people expect, and by the time there’s real pain, decay has usually already reached the more sensitive layers of the tooth. Knowing what to actually watch for, and understanding why some cavities never give you a warning, is the best defense you have between visits.

The Honest Answer: Early Cavities Often Feel Like Nothing

When decay first begins, during demineralization, before an actual hole has formed, there’s usually no pain and nothing to feel. That’s exactly why routine evaluation matters more than waiting for a symptom to show up. By the time you feel something, decay has typically moved past the enamel and into the dentin underneath, which is far more sensitive.

Signs and Symptoms Worth Paying Attention To

Once decay does start to make itself known, these are the signs most people notice first:

  • Sensitivity to hot, cold, or sweet foods that lingers after the trigger is gone
  • A toothache, whether dull and occasional or sharp and sudden
  • Pain when biting down, which can mean decay is affecting a specific spot under pressure
  • Visible staining, white, brown, or black, on the surface of a tooth (see our guide on what a cavity actually looks like at each stage)
  • A hole or rough spot you can feel with your tongue or see in the mirror
  • Persistent bad breath or an off taste that doesn’t go away with brushing (though this can also point to gum disease, worth mentioning at your visit either way)

Any one of these on its own is worth a look. Several together usually means decay has been present for a while.

Why Cavities Form in the First Place

A cavity isn’t random bad luck, it’s the result of an imbalance. Harmful bacteria, particularly Streptococcus mutans, feed on sugars left behind in the mouth and produce acid as a byproduct. When oral pH drops low enough, that acid begins pulling minerals out of the tooth faster than saliva and diet can replace them. Over time, enough mineral loss creates a true cavity.

The Cavities You Won’t Feel Until It’s Too Late

Some of the most common cavities form in places you simply can’t monitor yourself. The type and location matter, and knowing the pattern helps explain why some go unnoticed far longer than others:

  • Pit and fissure cavities form in the deep grooves of your back molars, spots that are hard to clean thoroughly even with good brushing, and hard to see even when you look.
  • Smooth surface cavities develop on the flatter sides of teeth. These tend to progress slowly, which actually gives more opportunity to support remineralization before a true cavity forms, if they’re caught early.
  • Interproximal cavities, the ones between teeth, are essentially invisible from the outside. No amount of careful inspection in the mirror will catch these.
  • Root or gumline cavities form where the tooth meets the gum, often when gum recession has exposed a surface that isn’t protected by enamel the way the rest of the tooth is.

These hidden cavities are exactly why we don’t rely on a visual check alone.

What Happens If a Cavity Goes Untreated

Left alone, decay doesn’t stay contained. It progresses from the enamel into the dentin, and eventually into the pulp, where the tooth’s nerves and blood vessels live. At that point, pain typically becomes much harder to ignore, and untreated decay can lead to infection or a dental abscess, a serious condition that requires urgent care. Catching decay early isn’t just more comfortable, it’s what keeps treatment minimally invasive in the first place.

Can a cavity have zero symptoms?

Yes, this is actually the most common stage. Early decay, before a true cavity has formed, typically causes no pain or sensation at all, which is why regular evaluation catches far more than waiting for symptoms ever will.

Is tooth sensitivity always a cavity?

Not always, sensitivity can also come from gum recession, enamel wear, or a cracked tooth. But lingering sensitivity to hot, cold, or sweets is common enough with cavities that it’s always worth having checked.

How fast does a cavity get worse?
